The work goes well beyond forwarding resumes. A specialized agency defines the role with the hiring team, sources candidates directly rather than waiting on applications, verifies state licensure and board certification, manages interviews and offer negotiation, and supports the candidate through credentialing so the start date holds.

That distinction matters, because most healthcare vacancies are not caused by a shortage of applicants. They are caused by the right candidate never hearing about the role, or hearing about it from someone who could not describe it credibly.

Licensure requirements, compact license eligibility, and credentialing timelines vary by state. Those variables get mapped at intake rather than discovered later, when they threaten a start date.

A staffing agency supplies temporary or contract workers who stay on the agency’s payroll. A recruiting agency finds candidates for permanent roles that sit on your payroll.

VHP supports both. Some situations call for contract or locum coverage to hold a unit together while a permanent search runs in parallel. Others call for a direct hire and nothing else. The right model depends on whether the gap is temporary or structural, which is worth an honest conversation before anything gets priced.

Direct-hire placement is generally billed as a percentage of the candidate’s first-year base compensation, with the rate varying by specialty and search difficulty. Retained executive search is milestone-based. RPO and contract recruitment are typically priced monthly or hourly.

VHP scopes and quotes every engagement in writing before work begins. On contingency direct-hire searches, there is no fee if the search does not result in a hire.

Contingency means the fee is owed only when a hire is made, with no exclusivity and no upfront commitment. Engaged search involves a partial fee up front with the balance due on placement. Retained search is exclusive and milestone-based, with dedicated research effort behind it.

Contingency fits roles with a reasonable candidate pool. Engaged fits hard-to-fill specialties and competitive or rural markets. Retained fits leadership roles, confidential replacements, and any search where a failed hire would be genuinely expensive.

Most firms push every engagement toward retained. VHP will tell you when a role does not require it.

Time to fill varies substantially by specialty, geography, and how competitive the compensation is. Primary care in a metro market moves faster than a subspecialist in a rural market, and physician searches routinely run longer than nursing or allied health searches because of the smaller candidate pool and the credentialing period that follows an accepted offer.

VHP gives a realistic, search-specific timeline at intake rather than quoting a national average that will not apply to your role.

Most stalled searches fail for one of five reasons, and all five are diagnosable at intake.

  • Off-market compensation for the geography. A package that works in one metro will not generate a single conversation two states away.
  • A generic pitch. “Competitive salary, collegial environment” describes several thousand openings. Candidates screen it out.
  • A slow interview process. Clinicians in active search are usually in three to five conversations at once. A two-week gap is where good candidates disappear.
  • A role defined by the vacancy rather than the need. Sometimes the seat that opened is not the seat that should be filled.
  • Passive sourcing. Posting and waiting reaches only the small share of the market that is actively looking.

A recruiter paid per placement has little incentive to raise any of this. VHP raises it in week two.

Scale and attention trade against each other, and VHP has chosen attention.

A national firm with dozens of offices can throw volume at a requisition. What it generally cannot do is give you a recruiter with 25 years of healthcare HR leadership who knows your organization, screens for cultural fit as rigorously as for credentials, and will tell you when the problem is not recruiting at all.

There is also a structural advantage. Large search firms cannot recruit from their own client organizations, which quietly removes a portion of the market from your search. A smaller firm has a wider field to work.
